Patient Forms

Upon entering our office, our Patient Care Coordinator will welcome you as a member of our practice family. You will have received our patient intake forms via email prior to your visit so please provide these completed forms as well as a copy of your insurance card. If you did not receive the forms, or do not have access to a printer, please arrive early to complete the forms in-office.   Copies of our patient forms are also located in the Patient Center. This paperwork provides us with your health history and information on your condition.

Consultation

Next, you will have a consultation with our chiropractor to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for our doctors to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

After your consultation, our chiropractor will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. Imaging, such as x-rays, will be performed or requested, if needed. However, nothing will be done in our office without your consent.

X-Ray & Imaging Studies

Depending on your specific condition, we may take X-rays as well. X-rays or other imaging may help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions.  We also may decide that a referral for advanced imaging such as a MRI or CT Scan may be appropriate following our examination.

Report of Findings

Once the information is collected, all examinations are performed and diagnosis and plan of care decision making has occurred, your chiropractor will give you a report of all findings and answer any questions including. Depending on your initial visit, this may occur on your first or second visit to our office:

1.  Can chiropractic help me?
2.  What is my diagnosis/health problem?
3.  How long will this take to get better and what is needed to be done?

After reviewing your health history, goals, and examining your spine and X-rays, Our doctors will discuss recommendations and notify you if your condition requires care with other providers.   Your chiropractor will provide the best treatment and wellness program for your needs.

Treatment

At the conclusion of the initial appointment, you may experience your first treatment. This may include spinal adjustments, but usually consists of pain and inflammation-centered treatments such as  physiotherapy modalities, and/or soft tissue massage. This decision is based on your chiropractor's treatment recommendations. Your full plan of care including spinal adjustments will begin only after your full Report of Findings so that you understand your health issue, diagnosis and specific plan of care.
